Students must provide the correct Kent State University email for the instructor in the instructor email column. If an incorrect instructor email is submitted, the form will not be processed. Forms that are deemed to be incomplete/incorrect may not be considered for review.
Late Adds/Re-adds:
This request is only for the CURRENT term courses that have a final grade of NF or verified as never attended. THIS FORM CAN ONLY BE USED DUE TO NEVER ATTENDING.
If this exception is approved, you will be officially withdrawn (without a W), as a result of never attending the course, resulting in a 100% tuition credit for the particular course(s).
Please note a tuition credit may not result in a credit on your student account.
